Health Homes Supporting Individuals with Intellectual and/or Developmental Disabilities (Care Coordination Organization/Health Homes (CCO/HH) CCO Provider Electronic Health Record Integration

The New York State Department of Health (DOH), in collaboration with the New York State Office for People with Developmental Disabilities (OPWDD), is working to expand the Health Home program to support individuals with intellectual and/or developmental disabilities (I/DD). Initial enrollment of individuals with I/DD in Health Homes began on July 1, 2018.

RHIO Consent Matrix
The purpose of this matrix is to provide guidance on which RHIO to select when an individual is filling out their consent form (i.e. DOH-5201, DOH-5055).

CCO Provider Electronic Health Record Integration

The following vendors have either completed certification or are working with MediSked to integrate their platforms with CCOs in the NY IDD CCO HH program:

  • Core Solutions
  • eVero

The functionality being integrated may include Enrollment, Life Plan Updates, Authorizations, Notices of Change, Charting, and/or Periodic Summaries.
